But for patients with more advanced periodontitis, where pockets are deep, bone has been significantly destroyed, and bacteria have established themselves in areas a cleaning instrument cannot reach, a deeper intervention is needed. That intervention is osseous surgery.

What Is Osseous Surgery?

Osseous surgery — also called pocket reduction surgery or flap surgery — is a periodontal surgical procedure in which the gum tissue is gently folded back to allow direct access to the roots of the teeth and the underlying bone. Once access is achieved, the periodontist removes bacterial deposits from the root surfaces, reshapes any irregular bone defects, and repositions the gum tissue to reduce pocket depth.

The goal is to create an environment that the patient can maintain through daily home care and regular professional maintenance — an environment where bacteria cannot accumulate in deep, unreachable pockets and continue destroying bone.


Who Needs Osseous Surgery?

Osseous surgery is typically recommended when:

  • Periodontal pockets are 5mm or deeper and have not responded adequately to deep cleaning

  • Bone defects are present that contribute to pocket depth and cannot be addressed with cleaning alone

  • Active bone loss is continuing despite non-surgical treatment

  • Irregular bone contours create areas where bacteria are impossible to remove with instruments

A comprehensive periodontal evaluation — including pocket depth measurements and bone assessment — determines whether surgery is indicated for a given patient.


What to Expect from the Procedure

Osseous surgery is performed under local anesthesia, and most patients are comfortable throughout. The gum tissue around the affected teeth is reflected (gently folded back), allowing the periodontist full visibility of and access to the tooth roots and bone.

After thorough cleaning of the root surfaces and reshaping of bone irregularities, the tissue is carefully sutured back into position at a reduced pocket depth. The entire procedure for a quadrant (one quarter of the mouth) typically takes sixty to ninety minutes.

Recovery involves mild to moderate discomfort for several days, managed with prescribed medication. Patients are typically able to return to normal activities the following day, with dietary modifications for one to two weeks.


Osseous Surgery with Bone Regeneration

In some cases, osseous surgery is combined with bone grafting and guided tissue regeneration — placing graft material and a protective membrane in bone defects to encourage the body to regenerate lost bone and ligament attachment. This combined approach can be highly effective for certain defect geometries and may improve the long-term stability of affected teeth.


Frequently Asked Questions

Is osseous surgery painful?

The procedure itself is painless due to local anesthesia. Post-operative discomfort is typically well controlled with prescribed pain medication. Most patients rate the experience as less uncomfortable than anticipated.

How long before I see results?

Pocket depths are re-evaluated approximately six to eight weeks after surgery. Patients typically see significant improvement in pocket depth measurements, reduced bleeding, and stabilization of bone levels over the following months.

Will I need surgery again in the future?

Surgery is not a permanent cure for periodontal disease. The bacteria responsible will continue to accumulate unless controlled through excellent home care and regular periodontal maintenance visits. Patients who maintain their maintenance schedule rarely require additional surgery.

Can osseous surgery save teeth that might otherwise be lost?

In many cases, yes. Teeth that appear hopeless without intervention can often be maintained for many additional years following successful osseous surgery combined with a committed maintenance program.
