The short answer is: when bone loss has occurred, grafting is often the essential first step that makes everything else possible.

Why Does Jawbone Loss Happen?

The jawbone around your teeth is maintained by the stimulation provided by tooth roots. When a tooth is lost — whether due to extraction, decay, or advanced gum disease — the bone in that area no longer receives stimulation and begins to resorb, or shrink, over time. This process begins within weeks of tooth loss and can be significant within the first year.

Bone loss also occurs as a direct consequence of untreated periodontal disease. The bacteria responsible for gum disease produce toxins that trigger an inflammatory response, and that inflammation gradually destroys the bone surrounding the teeth.


What Is a Bone Graft?

A bone graft is a surgical procedure in which bone material is placed in an area of deficiency to stimulate new bone growth and restore lost volume. Over several months, your body incorporates the graft material and produces new bone in its place — rebuilding the foundation needed to support an implant or stabilize existing teeth.

There are several types of bone graft material used in periodontal and implant surgery:

  • Autograft: bone taken from the patient's own body — the gold standard for integration

  • Allograft: donor bone from a tissue bank, processed and sterilized

  • Xenograft: bone derived from animal sources, most commonly bovine (cow) bone

  • Alloplast: synthetic bone substitute materials

The choice of graft material depends on the size and location of the defect, the planned treatment, and patient-specific factors. Dr. Liu discusses the options with each patient individually.


When Is Bone Grafting Needed?

Before Dental Implants

Implants require a minimum volume of bone to be placed successfully and integrate properly. If a tooth has been missing for a period of time, or was lost due to bone-destroying infection, grafting is often required to build up the site before implant placement can proceed.

After Tooth Extraction (Socket Preservation)

A socket preservation graft is placed immediately at the time of extraction to minimize bone loss before the implant is placed. Without socket preservation, significant bone resorption can occur in just the first few months, potentially complicating future implant placement.

As Part of Periodontal Surgery

In cases of advanced periodontitis where bone loss has created defects around existing teeth, bone grafting combined with guided tissue regeneration can rebuild lost support and improve the long-term outlook for those teeth.


What to Expect from the Procedure

Bone grafting is performed under local anesthesia and is typically well tolerated. The graft material is placed in the deficient area, often covered with a membrane to protect it during healing, and the site is sutured closed. Healing time varies by graft size but typically ranges from three to six months before the grafted site is ready for implant placement.

Post-procedure discomfort is generally mild to moderate and managed well with prescribed medication. Most patients return to normal activity within a few days.


Frequently Asked Questions

Is bone grafting painful?

The procedure is performed under local anesthesia, so you will not feel pain during surgery. Mild to moderate soreness and swelling in the days following are normal and manageable with medication.

How long does it take for a bone graft to heal?

Most grafts require three to six months to mature sufficiently for implant placement. Your periodontist will monitor healing with follow-up imaging before proceeding.

Can bone grafting fail?

Graft failure is uncommon in healthy patients but can occur. Risk factors include smoking, uncontrolled diabetes, certain medications, and infection. We screen for these factors during your consultation and provide detailed post-operative instructions to maximize success.

Do I need a bone graft if I want an implant but just had the tooth extracted?

Not always — but often a socket preservation graft at the time of extraction is recommended to preserve bone volume and simplify future implant placement. This is assessed on a case-by-case basis.
