Gum Recession: Causes, Risks, and Treatment Options
Gum recession — the gradual exposure of tooth roots as gum tissue pulls back — is one of the most common yet underappreciated dental problems. Many patients first notice it as tooth sensitivity to cold or hot foods, or as teeth that appear longer than they once did. What may seem like a cosmetic concern is often a sign of a deeper problem that, left untreated, can lead to root decay, bone loss, and eventually tooth loss.
At Pleasant Dental Associates, Dr. Jerry Chiu evaluates and treats gum recession regularly, helping patients understand the underlying cause and the most effective path to correction.
What Causes Gum Recession?
Gum recession rarely has a single cause. The most common contributing factors include:
Periodontal disease — the most significant cause; bacterial infection destroys gum and bone tissue
Aggressive brushing — scrubbing too hard or using a hard-bristled brush wears away gum tissue over time
Genetics — some patients are born with naturally thin or fragile gum tissue that is more susceptible to recession
Teeth grinding (bruxism) — the forces generated by grinding put excessive stress on the gum attachment
Misaligned teeth — teeth that are positioned outside the natural arch can have thin or absent bone over the root, making the gum more vulnerable
Piercings — lip or tongue piercings that repeatedly contact the gum can cause localized recession
Why Recession Should Not Be Ignored
When gum tissue recedes, it exposes the root surface of the tooth. Unlike enamel, root surfaces are not covered by a hard protective layer — they are vulnerable to decay, sensitivity, and wear. Recession also reduces the attachment that anchors gum tissue to the tooth, and once gum tissue is lost, it does not grow back on its own. Without treatment, recession tends to progress.
Beyond individual tooth health, widespread recession is often a visible sign of underlying periodontal disease that requires evaluation and treatment of the infection before the recession itself is addressed.
Treatment Options for Gum Recession
Gum Grafting (Connective Tissue Graft)
The most established and effective treatment for gum recession is a connective tissue graft, in which a small amount of tissue is taken from the roof of the mouth (palate) and used to cover the exposed root and rebuild the gum level. The procedure is performed under local anesthesia, and results are predictable and long-lasting when performed by an experienced periodontist.
Pinhole Surgical Technique
A minimally invasive alternative to traditional grafting, the pinhole technique involves making a small access point in the gum tissue and gently repositioning existing tissue over the exposed roots. It involves no donor site and typically results in faster recovery. Your periodontist will advise whether this is appropriate for your case.
Treating the Underlying Cause
In all cases, correcting the underlying cause of recession is essential. This may include treating active periodontal disease, adjusting brushing technique, recommending a night guard for grinding, or discussing orthodontic options for misalignment.
Frequently Asked Questions
Will my gums grow back on their own?
No. Once gum tissue has receded, it will not regenerate without surgical intervention. This is one of the key reasons not to wait — recession typically progresses rather than stabilizing.
Is gum grafting painful?
The procedure is performed under local anesthesia. The donor site at the palate is typically the source of any post-operative tenderness, which resolves within one to two weeks. Most patients find the recovery more manageable than expected.
Is gum recession a sign of gum disease?
It can be, but not always. Recession caused by aggressive brushing or thin tissue type may occur without active infection. A comprehensive periodontal evaluation will determine the cause and guide treatment.
Can recession affect implants?
Yes. Recession around implants — called peri-implant recession — can expose the implant surface and affect both function and aesthetics. Grafting around implant sites is a service we provide.
